Granulomatous Phlebitis of the Hepatic Vein Tributaries after Portal Venous Embolization

چکیده
Background & Aims: Granulomatous phlebitis of hepatic vein is a rare disease and its exact etiopathogenesis remains unknown. Recently, we experienced granulomatous phlebitis of small hepatic veins in a patient who underwent hepatic resection for metastatic liver cancer after preoperative portal venous embolization (PVE). Methods: We surveyed such phlebitis in the patients who underwent PVE (2008 August – 2014 August). A total of 62 patients underwent PVE followed by hepatectomy for hepatocellular carcinoma (HCC), peripheral cholangicarcinoma (pCCA), or metastatic liver tumor (MLT) during this period. As a control, 20 cases of surgically resected livers without a history of preoperative PVE for liver tumors of comparable age and sex distribution in recent 2 years in the same hospital, were used. Results: Granulomatous phlebitis of small hepatic veins was found in 10 patients with a history of PVE (16.1% of 62 cases) but not in control cases. No microbial agents were identified in these granulomatous lesions. Interestingly, the presence of embolic materials containing sponge gel with inflammation and foreign body reaction in the portal veins in the embolized livers were independently associated with granulomatous phlebitis. Conclusions: It was suggested that sponge gel used for PVE might have be directly or indirectly related to the occurrence of granulomatous phlebitis. This is the first report of granulomatous phlebitis related to PVE.

Extrahepatic Portal Hypertension following Liver Transplantation: a Rare but Challenging Problem
This study reports our experience of 8 cases of extrahepatic portal hypertension after 273 orthotopic liver transplantations in 244 adult patients over a 10-year period. The main clinical feature was ascites, and the life-threatening complication was variceal bleeding. Extrahepatic portal hypertension was caused by portal vein stenosis in 6 patients, and left-sided portal hypertension in 2 pati...
متن کامل"Pants" vein jump graft for portal vein and superior mesenteric vein thrombosis in transplantation of the liver.
PORTAL VEIN TlIROMBOSIS (PVT) or thrombosis of its tributaries has been a formidable surgical challenge in orthotopic hepatic transplantation (OHT). At one time, PVT was considered a relative contraindication for OHT. However, refinements of the technique to reconstruct the portal vein after thrombosis have included venous grafting procedures (1, 2). The technique of the venous "jump" graft fro...
متن کاملHepatic outflow obstruction created by balloon occlusion of the hepatic vein: induced hepatic hemodynamic changes and the therapeutic applications of hepatic venous occlusion with a balloon catheter in interventional radiology.
Hepatic outflow obstruction created by balloon occlusion of the hepatic vein induces characteristic angiographic findings in the occluded area: prolonged enhancement on hepatogram followed by reversed portal opacification on the hepatic arteriogram and perfusion defect on the arterial portogram. The following induced hepatic hemodynamic changes are suggested: hepatic arterial flow increases, an...
متن کاملCombination therapy using PSE and TIO ameliorates hepatic encephalopathy due to intrahepatic portosystemic venous shunt in idiopathic portal hypertension
A 64-year-old woman treated for anemia and ascites exhibited hepatic encephalopathy. Abdominal ultrasonography and computed tomography (CT) showed communication between the portal vein and the middle hepatic vein, indicating an intrahepatic portosystemic venous shunt (PSS). Since hepatic encephalopathy of the patient was resistant to medical treatment, interventional radiology was performed for...
متن کاملA comparison of portal vein embolization with poly(2-hydroxyethylmethacrylate) and a histoacryl/lipiodol mixture in patients scheduled for extended right hepatectomy.
To determine whether PHEMA [poly(2-hydroxyethylmethacrylate)] is suitable for portal vein embolization in patients scheduled to right hepatectomy and whether it is as effective as the currently used agent (a histoacryl/lipiodol mixture). Two groups of nine patients each scheduled for extended right hepatectomy for primary or secondary hepatic tumor, had right portal vein embolization in an effo...
